Health and Wellbeing Coaches

Working collaboratively with Southampton PCN, Southampton Primary Care Ltd have set up Health and Wellbeing Coaching service to our patients achieve sustainable lifestyle change. Health and Wellbeing coach will help you make positive lifestyle changes that will help you become the healthiest, happiest version of you. It is a person-centered approach, focusing on what matters to you and aims to help create lasting change to improve a person’s wellbeing and overall health. They encourage people to take ownership of their goals and to feel more confident and motivated to manage their health.

How do I access a Health and Wellbeing coach?

You can be referred to a Health and Wellbeing coach by any healthcare professional at our practice.

You will then receive a phone call from the reception team, and they will book in your first in-person session. Subsequent sessions can be conducted in-person, virtual and/or via phone call.

  • An initial consultation, where your coach will find out more about you, your lifestyle, values, goals and anything that has the potential to be a barrier against achieving these goals. You will then set short-term goals that will enable you to work towards what it is that you want to achieve.
  • Following this session, you will work together on identifying and discussing expectations and the steps to support you on your journey towards feeling more empowered by your health.
  • You will have a further five sessions in which you will review your progress towards your targets, set out any new goals and discuss any setbacks that you may have faced. Together with your Health and Wellbeing coach, you can plan how to work around those setbacks should they arise again in future.
  • Your final session is a review which allows you and the coach to reflect on your progress so far. At this point you will decide whether you feel that you would benefit from more sessions or if you feel that you can continue your health journey on your own, knowing of course that you can always return to the coach in future should you need to.

If you feel you require more support, it may be possible to book a block of four further sessions, followed by a final review, until you and your coach feel you no longer need the support of the coach.
