Self Help

Health and Wellbeing Coaches: information about service jointly provided by Southampton Primarycare Care Limited and our local PCN (Southampton West PCN) - aiming to support you in achieving your health goals. Please contact us for further information

Connect to Support - Hampshire: explore resources available locally to help people look after yourself, stay safe and connected with your community. You are welcome to use Information Finder to explore helpful resource if you are not sure what to look for. 

You are also welcome to visit Personalised care website to learn about health conditions and services available in Hampshire to help you improve your health. 

Digital Resources for Self Help: provides a collection of digital resources to help yourself or explore further information about your health conditions. Health Care Videos provide useful collection of health and care videos offering information about a range of health conditions and what you can do to manage them yourself. The video library is also available as an App. 

Your Health Collect 2022 is an online collection of tools and digital books to support you to live well and enjoy better health. All are free to access. Books can be borrowed from NHS libraries across England and anyone can access the online collection at their leisure via any computer or mobile device.  Find out more from your local NHS service.

Health and Wellbeing resources page on Hampshire and Isle of Wight People Portal is another useful resoruce to explore opportunities to support your health and wellbeing. 

Healthy living and life style: for advice and help to improve life style and habits - quit smoking, drink less, lose weight etc

Health 4 Her Programme: Health 4 Her (H4H) is a weight loss programme designed to provide an appropriate and safe exercise environment to facilitate an increase in physical activity participation for Black, Asian, and Minority Ethnic women from low socio-economic areas of Southampton. H4H is currently being delivered online via a Facebook group but will return to face to face delivery when is it safe to do so and will adhere to the government guidelines laid out at that time.

Weight Management

Southampton City Council Integrated Commissioning Unit and Public Health have commissioned Solutions 4 Health, to help Southampton residents lose weight and introduce healthy living. The service is FREE to Southampton residents who are age 18+, who have BMI over 30 and registered with a GP. The service ends on 30th September 2022.

Community Support

 Senior Saints - Active Life Style Support Programme: Saints Foundation provides support for active life style and falls recovery to help improve active life style and strength to prevent falls. Sessions are offered daily throughout the year, with the helpf of foundation staff on hand to provide assistance and expertise. They are currently running a blended programme of online delivery alongside some outdoor walking sessions. Senior Saints Strolls are for anyone over the age of 60, all at a moderate level and between 2 to 3km. Please call 02380718605 for further information

Social and Community Support: information regarding support networks, voluntary organisations who can offer help if needed.

Scratch: a registered charity dedicated to relieve the effects of poverty for families and individuals with needs recognised by approved referral agencies in Southampton and surrounding areas. Located in Southampton, they provide assistance to families and individuals in Southampton and across Hampshire.
